目的探讨中性粒细胞/淋巴细胞比值(neutrophil to lymphocyte ratio,NLR)与24h尿微量白蛋白排泄率(urine albumin excretion rate,UAER)的关系。方法选取2015年7—12月在该院就诊的107例2型糖尿病患者,根据UAER水平将患者分为糖尿病肾病(diabetic nephropathies,DN)组和非DN组,收集患者的一般临床资料及生化指标进行统计学分析。结果 DN组中性粒细胞计数、糖化血红蛋白A1c、NLR均显著高于非DN组(4.30±1.47 vs 3.58±1.01,P=0.028;9.53%±1.99%vs 8.53%±1.94%,P=0.034;2.16±0.81 vs 1.69±0.81,P=0.018)。NLR与UAER(r=0.337,P=0.003)、HbA1c(r=0.239,P=0.018)正相关。回归分析显示NLR(OR=1.854,95%CI:1.006~3.417,P=0.048)、HbA1c(OR=1.333,95CI:1.026~1.733,P=0.031)、糖尿病病程(OR=1.179,95CI:1.019~1.363,P=0.026)是DN的独立危险因素。结论 NLR在DN患者中显著升高,与UAER显著相关。NLR是DN的独立危险因素之一。
Objective To investigate the relationship between neutrophil to lymphocyte ratio (NLR) and urine albumin excretion rate (UAER). Methods A total of 107 type 2 diabetic patients treated in our hospital from July to December in 2015 were divided into diabetic nephropathies (DN) group and non-DN group according to UAER level. The clinical data and biochemical indexes Statistical analysis. Results The neutrophil count, A1c and NLR in DN group were significantly higher than those in non - DN group (4.30 ± 1.47 vs 3.58 ± 1.01, P = 0.028, P <0.05) 2.16 ± 0.81 vs 1.69 ± 0.81, P = 0.018). NLR was positively correlated with UAER (r = 0.337, P = 0.003) and HbA1c (r = 0.239, P = 0.018). Regression analysis showed that the incidence of diabetes mellitus (OR = 1.179, 95 CI: 1.019 ~ 0.994) was significantly higher than that of the controls (OR = 1.854,95% CI: 1.006-3.417, 1.363, P = 0.026) were independent risk factors for DN. Conclusion NLR was significantly increased in patients with DN, was significantly associated with UAER. NLR is one of the independent risk factors for DN.
