目的:评估口腔支架在头颈部放疗中保护舌的效果。方法:计算机检索Embase、Pubmed、Google scholar英文数据库和中国知网、万方、维普等中文数据库,筛选关于口腔支架在头颈部肿瘤放疗运用的相关随机对照试验,并用RevMan 5.2软件进行Meta分析。结果:最终纳入5个RCT,共203例患者。Meta分析结果示:口腔支架在头颈放疗中降低舌体平均照射剂量[MD=-11.89,95%CI=(-15.48,-8.30),P<0.00001],降低味觉损伤率[OR=0.12,95%CI=(0.05,0.28),P<0.00001]以及降低舌黏膜毒性反应率[OR=0.09,95%CI=(0.02,0.37),P=0.00009],差异均有统计学意义。结论:口腔支架在头颈部放疗中能有效保护舌体组织,基于纳入研究的局限性与可能存在的偏倚,需要更多严谨的大样本的研究予以论证。
OBJECTIVE: To assess the effectiveness of oral stents in protecting the tongue during radiotherapy of head and neck. Methods: The Chinese database of Embase, Pubmed, Google scholar, CNKI, Wanfang and VIP were searched by computer, and related randomized controlled trials on the use of oral stents in radiotherapy of head and neck cancer were screened. Meta-analysis was performed with RevMan 5.2 software. Results: Five RCTs were eventually enrolled, with a total of 203 patients. The results of Meta-analysis showed that the oral stent reduced the average dose of tongue in radiotherapy of head and neck (MD = -11.89,95% CI = (-15.48, -8.30), P <0.00001] and decreased the rate of taste damage [OR = 0.12,95 % CI = (0.05,0.28), P <0.00001], and decreased the rate of tongue mucosal toxicity [OR = 0.09,95% CI = (0.02,0.37), P = 0.00009], the difference was statistically significant. CONCLUSIONS: Oral stents effectively protect the tongue tissue during radiotherapy of head and neck. Based on the limitations of the study included and possible biases, more stringent large sample studies are warranted.
