目的应用彩色多普勒超声技术检测初诊2型糖尿病患者肱动脉血管内皮功能及颈动脉、足背动脉粥样硬化的状况并探讨其相关性。方法对50例初诊2型糖尿病患者与对照组非2型糖尿病患者30例,超声检查颈动脉、足背动脉及测定反应性充血时和舌下含服硝酸甘油后肱动脉内径的变化,并对其进行相关性分析。结果糖尿病组和对照组,在加压和口服药的情况下与静息状态下相比血管内径均增大,但糖尿病组内径变化率均明显低于对照组(P<0.01,P<0.05),差异有统计学意义。糖尿病组颈动脉、足背动脉IMT较对照组明显增厚,差异有统计学意义(P<0.01)。糖尿病组颈动脉异常率、足背动脉异常检出率明显高于对照组;糖尿病组足背动脉异常检出率明显高于颈动脉异常。结论采用彩色多普勒超声技术联合检测肱动脉皮功能及颈动脉、足背动脉粥样硬化是评价糖尿病血管异常的有效方法,对早期诊断、预防及治疗具有重要的临床应用价值。
Objective To evaluate the relationship between brachial artery endothelial function and carotid and dorsal carotid atherosclerosis in newly diagnosed type 2 diabetic patients by color Doppler ultrasonography. Methods 50 patients with newly diagnosed type 2 diabetes mellitus and 30 non-type 2 diabetes mellitus patients were included in this study. The carotid and dorsalis pedis artery were examined ultrasonically and the brachial artery diameter was measured after sublingual nitroglycerin was administered. It conducts correlation analysis. Results Diabetic group and control group showed a significant increase in intravascular diameter compared with those in resting state under pressure and oral administration, but the rate of change of inner diameter of diabetic group was significantly lower than that of control group (P <0.01, P <0.05) ,The difference was statistically significant. The IMT of carotid artery and dorsalis pedis artery in diabetic group was significantly thicker than that in control group (P <0.01). The detection rate of carotid artery abnormality and dorsalis pedis artery in diabetic group was significantly higher than that in control group. The detection rate of abnormalities of dorsalis pedis artery in diabetic group was significantly higher than that of carotid artery. Conclusion The combined detection of brachial artery function, carotid artery and dorsalis pedis artery atherosclerosis by color Doppler ultrasound is an effective method to evaluate the vascular abnormalities in diabetic patients. It has important clinical value in early diagnosis, prevention and treatment.
