目的探讨慢性肺部疾病患者接种23价肺炎链球菌多糖疫苗(PPV)后血清中抗肺炎链球菌荚膜多糖(CPS)IgG抗体的特异性。方法以39例慢性肺部疾病患者为研究对象,分别用第二代和特异性更高的第三代ELISA测定疫苗接种前、后血清中的肺炎链球菌6B、19F和23F三种血清型的特异性IgG抗体浓度。结果用第三代ELISA测得的患者血清中血清型特异性IgG抗体水平比用第二代ELISA测得的抗体水平显著降低。无论是PPV接种前的自然感染,还是PPV接种后的主动免疫,慢性肺部疾病患者血中产生的抗CPS-IgG中都有大约一半的非特异性抗体。结论 PPV接种并不能提高慢性肺部疾病患者血中的特异性抗CPS-IgG的比例。
Objective To investigate the specificity of anti-S. pneumoniae capsular polysaccharide (CPS) IgG in serum of patients with chronic lung disease after inoculation of 23-valent pneumococcal polysaccharide vaccine (PPV). Methods Thirty-nine patients with chronic pulmonary disease were studied. The second-generation and third-generation specific ELISA were used to determine the serotypes of S. pneumoniae 6B, 19F and 23F before and after vaccination Specific IgG antibody concentration. Results Serum-specific IgG antibody levels in patient sera measured with the third-generation ELISA were significantly lower than those detected with the second-generation ELISA. About half of the non-specific anti-CPS-IgG antibodies produced in the blood of patients with chronic lung disease are either naturally infected before PPV vaccination or active immunization after PPV vaccination. Conclusions PPV vaccination does not increase the proportion of specific anti-CPS-IgG in the blood of patients with chronic lung disease.
