脑益嗪和氟苯桂嗪(flunarizine)为选择性钙通道阻滞剂,具有抗组织胺、抗5羟色胺及抗多巴胺能活性,临床广泛用作迷路镇静剂及血管扩张剂。常见的副反应为乏力和嗜睡,而锥体外系副反应少见。作者报道15例服药引起锥体外系副反应。15例中男4、女11例,年龄37～82(平均67.4)岁。全部均因眩晕、高血压等服氟苯桂嗪(8例)、脑益嗪(5例)或两药联合服用(2例)。剂量均为治疗剂量,其中氟苯桂嗪10mg/d8例,11.5mg/d2例;脑益嗪75mg/d1例,150mg/d4例,225mg/d2例。服药4小时至4年出现副反应,其中<6个月者8例,>6个月者7例。无1例服其他已知可引起异常运动的药物。锥体外系副反应包括:帕金森病综合征11例,其中1例并伴持久性静坐不能及口面运动困难;余4例为持久性静坐不能伴口面运动困难、单纯持久性静坐不能、急性肌张力障碍、面肌震颤各1例。7例伴有抑郁症,多为帕金森综合征患者。6例帕金森综合征及全部抑郁症患者用左旋多巴及/或溴隐亭或抗抑郁剂治疗均无效。14例停服氟苯桂嗪和脑益嗪后,症状改善。帕金森综合征消失时间为15～180(平均67)天。恢复时间与出现症状时的年龄、服药时间、剂量及症
