目的:报道1993年1月~1996年10月收治平时四肢火器伤57例,旨在探讨不同初期外科处理方法对其愈合及功能恢复的影响。方法:四肢火器伤57例,其中23例(A组)按传统方法行彻底清创,延期缝合;21例(B组)单纯切开,引流;12例(C组)清创Ⅰ期缝合。结果:A组伤平均愈合时间26天,骨折平均愈合时间12周,2例感染。B组伤口平均愈合时间15天,骨折平均愈合时间12周,无感染病例。C组伤口平均念合时间18天,骨折平均愈合时间4周,2例感染。5例血管损伤和11例神经损伤均Ⅰ期愈合。结论:根据临床观察和统计学分析,三种方法中,切开引流法更为安全,简单,有效,并具广泛的适应证。
OBJECTIVE: To report the clinical data of 57 cases of firearms in normal limbs from January 1993 to October 1996, and to investigate the effects of different initial surgical treatments on their healing and functional recovery. Methods: Fifty-seven cases of firearm wounds on the extremities were involved. Thirteen patients (group A) underwent radical debridement and sutured debridement by traditional methods. Twenty-one patients (group B) underwent simple incision and drainage, and 12 patients (group C) debridement and primary suture. Results: In group A, the average wound healing time was 26 days, the average fracture healing time was 12 weeks, and 2 cases were infected. In group B, the mean wound healing time was 15 days and the average healing time of fracture was 12 weeks, with no infection. The average wound healing time of group C was 18 days, the average healing time of fracture was 4 weeks, and 2 cases were infected. Five cases of vascular injury and eleven cases of nerve injury were all healed. Conclusion: According to clinical observation and statistical analysis, among the three methods, incision and drainage is safer, simple and effective, and has a wide range of indications.
