目的 :探讨带蒂筋膜瓣管桥接家兔面神经缺损后面神经再生效果及机制 ,旨在寻找一种能替代自体神经移植的非神经组织材料。方法 :实验采用2 4只家兔 ,随机分为两组。 A组游离面神经 ,切断 1 0 mm神经段 ,腮腺咬肌筋膜缝合成管状 ,将两神经断端置入管中 ,间距 1 0 mm,在筋膜瓣管中注入 0 .5ml生理盐水。B组做神经原位移植术。术后饲养 1 2周 ,观察功能恢复情况 ,对移植体进行神经电生理、组织学和免疫组织化学检测。结果 :实验结果证明再生神经纤维都可通过桥接体。恢复正常进食时间和眨眼反射出现时间 ,两组相近 ,NCV、NAP、轴突直径、髓鞘厚度 ,A、B组无显著性差异。结论 :腮腺咬肌筋膜瓣管可作为替代自体神经移植修复面神经缺损的材料。
Objective: To explore the effect and mechanism of pedicled fascia flap bridge on facial nerve regeneration after facial nerve defect in rabbits, aiming at finding a non-nerve tissue material that can replace autologous nerve graft. Methods: Twenty-four rabbits were randomly divided into two groups. A group of facial nerve free, cut off 10mm nerve segment, the parotid muscle fascia sutured into the tube, the two ends of the nerve into the tube, spacing 10mm, in the fascia valve tube injected 0.5ml saline. Group B underwent neuro otopic implantation. The rabbits were kept for 12 weeks after operation, and functional recovery was observed. Electroencephalography, histology and immunohistochemistry were performed on the grafts. Results: The experimental results show that regenerated nerve fibers can pass through the bridge. Restoration of normal eating time and blink reflex appearance time, similar to the two groups, NCV, NAP, axonal diameter, myelin thickness, A, B group no significant difference. Conclusion: Parotid muscle fascia flap can be used as an alternative to autologous nerve graft to repair facial nerve defects.
