目的:探讨磁敏感加权成像技术在脑血管畸形诊断中的价值。方法:经临床及影像检查证实的36例脑血管畸形患者,行头颅MRI横轴面T1WI、T2WI、SWI扫描。比较各序列对脑血管畸形病灶的检出及表现,评价SWI在脑血管畸形检查和诊断中的作用。结果:SWI分别检出海绵状血管瘤、发育性静脉畸形和AVM病灶73个、8个、10个,明显高于T1WI和T2WI检出的病灶数,且病灶边界显示更加清晰,发现更多、更小的病灶。在SWI图像上海绵状血管瘤表现为大小不等的明显低信号,发育性静脉畸形表现为一条边界清晰低信号影,显示更多细小静脉向引流静脉集中呈星芒状,AVM表现为异常粗大的血管迂曲扩张,呈明显的低信号。结论:SWI对脑血管畸形的检出具有极高的敏感性,能够检出更多、更小病灶,且病灶显示更加清晰,可作为脑血管畸形检查的常规。
Objective: To investigate the value of magnetic susceptibility weighted imaging in the diagnosis of cerebrovascular malformations. Methods: Thirty-six patients with cerebrovascular malformation confirmed by clinical and imaging examination were scanned with T1WI, T2WI and SWI scan of cranial MRI. The detection and manifestation of cerebrovascular malformation lesions were compared among various sequences, and the role of SWI in the examination and diagnosis of cerebrovascular malformations was evaluated. Results: 73, 8 and 10 AVM lesions detected by SWI were significantly higher than those detected by T1WI and T2WI, respectively, and the border of the lesions showed more clearly and found more, Smaller lesions. In SWI images, cavernous hemangiomas showed markedly low signal of varying sizes. Developmental venous malformations showed a clear and low signal edge, which showed more venous veins to star-shaped veins. The AVM was abnormally thick The tortuous vascular expansion, showed a clear low signal. Conclusion: SWI is highly sensitive to the detection of cerebrovascular malformations. It can detect more and lesser lesions, and the lesions show more clearly, which can be used as a routine examination of cerebrovascular malformations.
