目的 :探讨手法旋转胎头对降低头位难产的作用。方法 :对 16 8例子宫口扩张 7~ 10cm、胎头位呈持续性枕横位的产妇 ,施行手法旋转 ,LOT逆时针旋转 90° ,ROT顺时针旋转 90°为枕前位。结果 :16 8例中 ,12 3例手法旋转后自然分娩 ,成功率为73.2 1% ,45例手术助产 (产钳助产加剖宫产 ) ,失败率为 2 6 .79%。胎儿体重≤ 35 0 0g者转位成功率显著高于胎儿体重 >35 0 0g者 ,两者差异有显著性 (P <0 .0 0 5 )。结论 :手法旋转持续性枕横位可显著降低头位难产率 ,减少产妇痛苦 ,降低手术助产并发症的发生率 ,有利于保障母婴健康。
Objective: To explore the effect of manual manipulation of fetal head to reduce head dystocia. Conclusion: The rotation of continuous occipital transverse approach can significantly reduce the head dystocia rate, reduce maternal pain and reduce the incidence of surgical complications of births, help to protect the health of mothers and children.
