目的了解我院门诊疑似患者人乳头瘤病毒(HPV)感染情况及其基因亚型分布情况。方法回顾性分析2014年1月-2015年12月本院门诊疑似HPV感染者临床资料及其基因亚型检测结果,比较不同性别、季节和年龄组HPV感染特点及其基因亚型分布情况。结果 9 154例门诊疑似HPV感染患者中,HPV阳性2 125例(23.21%),其中男93例(4.38%),女2 032例(95.62%)。低危亚型以HPV 6和HPV81检出率最高,男分别为15.56%和8.89%,女分别为16.42%,11.58%;高危亚型以HPV52,16和58型检出率最高,男分别为22.73%,16.67%和12.02%,女分别为20.42%,14.74%和8.11%。31~40岁为感染最高峰,21~50岁组HPV检出率明显高于其他年龄组HPV检出率,差异均有统计学意义(P均<0.05)。单一HPV亚型感染1 416例(72.99%),高危型HPV感染1 268例(65.36%),HPV感染主要流行于春季(尤其3月和4月)与夏季(特别是7月和8月)。结论我院门诊就诊人群HPV感染率较高,以中青年女性为著,呈单一高危型HPV感染,应提高疑似HPV感染高危人群季节性筛检力度与范围。
Objective To understand the prevalence of human papillomavirus (HPV) inpatients and the distribution of their subtypes in our hospital. Methods The clinical data and genotypes of HPV inpatients with suspected HPV infection from January 2014 to December 2015 in our hospital were retrospectively analyzed. The characteristics of HPV infection and the distribution of their genotypes were compared among different sexes, seasons and age groups. Results Of the 9 154 outpatients with suspected HPV infection, 2 125 (23.21%) were positive for HPV, of which 93 (4.38%) were male and 2 032 (95.62%) were female. Low-risk subtypes had the highest detection rate of HPV 6 and HPV81, with 15.56% and 8.89% for males and 16.42% and 11.58% for females respectively. The highest detection rates were HPV52, HPV16, and 58 in high-risk subtypes 22.73%, 16.67% and 12.02%, respectively, and women were 20.42%, 14.74% and 8.11% respectively. The highest infection rate was seen in 31-40 years old. The detection rate of HPV in 21-50 years old group was significantly higher than that in other age groups (all P <0.05). A total of 1 416 cases (72.99%) were infected with a single HPV subtype and 1 268 (65.36%) with a high risk HPV infection. The prevalence of HPV infection was mainly in spring (especially in March and April) and summer (especially in July and August) . Conclusions The prevalence of HPV infection in outpatient clinics in our hospital is relatively high, with young and middle-aged women as a single high-risk HPV infection. The intensity and scope of seasonal screening should be improved for those at high risk of HPV infection.
