目的 :采用 2 4小时动态血压监测方法对照评价硝苯地平控释片和依那普利缓释片的降压疗效及其对代谢指标及尿白蛋白的影响。方法 :6 0例 2型糖尿病合并高血压患者随机分为治疗组 30例给予硝苯地平控释片 ,剂量 30 mg~ 90 mg一日一次 ,对照组30例服用依那普利缓释片 ,剂量 1 0 m g~ 40 mg一日一次 ,治疗 6周。治疗前后均查 2 4小时动态血压、血糖、血脂、糖化血红蛋白、血尿酸、尿白蛋白指数。结果 :硝苯地平控释片和依那普利缓释片组均有降压作用 ,但硝苯地平控释片组较依那普利缓释片组能明显降低 2 4hSBP、d SBP、夜间血压、血压负荷值 (P<0 .0 5 ) ,其中血压负荷值降低更明显 (P<0 .0 1 ) ;能明显恢复昼夜血压节律 ;具有高T/ P比值 ;能明显降低尿白蛋白指数 (P<0 .0 5 ) ;能降低血尿酸。结论 :硝苯地平控释片是 2型糖尿病合并高血压患者的一种理想降压药物
OBJECTIVE: To evaluate the antihypertensive effects of nifedipine controlled release tablets and enalapril sustained-release tablets and their effects on metabolic indices and urinary albumin by 24-hour ambulatory blood pressure monitoring. Methods: Totally 60 patients with type 2 diabetes mellitus complicated with hypertension were randomly divided into treatment group (n = 30) and nifedipine controlled release (n = 30). The control group was given enalapril sustained- Dose 10 mg ~ 40 mg once daily for 6 weeks. Before and after treatment were checked 24 hours ambulatory blood pressure, blood glucose, blood lipids, glycosylated hemoglobin, serum uric acid, urinary albumin index. Results: Nifedipine controlled release tablets and enalapril sustained-release tablets were antihypertensive effect, but the controlled release nifedipine group compared with enalapril sustained-release tablets group was significantly lower 24 hSBP, d SBP, night Blood pressure and blood pressure (P <0.05). The blood pressure load decreased more obviously (P <0.01), the circadian rhythm of blood pressure could be recovered obviously, and the ratio of high T / P was high. The urinary albumin Index (P <0. 05); can reduce blood uric acid. Conclusion: Nifedipine GITS is an ideal antihypertensive drug for type 2 diabetes mellitus patients with hypertension
