结核病的防治是一项长期而艰巨的工程,在我国,投入了大量的人力、物力和财力、取得了一定效果。本文利用回顾性和前瞻性两种方法对广东省台山市结防所和惠阳市慢病防治站进行了结核病防治成本测算,以期用测算结果来评价防治效果。从对广东省1995年结核病控制项目的成本测算中可以看出,台山市结防所完成治疗一例新发涂阳病人成本为544.86元,完成治疗一例复治涂阳病人成本为653.03元;惠阳市慢病站完成治疗一例新发涂阳病人成本为743.49元,完成治疗一例复治涂阳病人成本为845.02元。乡镇卫生院结核病防治人员平均管治一例病人成本为164.92元。在1996年12月、1997年3、6、9月进行的4次前瞻性调查中,成本测算结果为:台山市结防所完成一例新发涂阳病人成本为327.76元,完成治疗一例复治涂阳病人成本为439.73元;而惠阳市慢病站完成治疗一例新发涂阳病人成本为675.27元,完成治疗一例复治涂阳病人成本为787.80元。乡镇卫生院结核病防治人员平均管治一例病人成本台山为62.29元,惠阳为226元。从成本——效果分析上来看,台山市结核病防所比惠阳市慢性病防治站资源使用情况好。建议结核病防治机构应建立结核病防治所(站)及科室二级成本核算制度;改革财务报表制度,以适应成本核算的需要;建立各科室各项工作原始记录登记制度;开展项目评估时应考虑经济因素,进行成本——效果综合评价。
Prevention and control of tuberculosis is a long-term and arduous project. In our country, a lot of manpower, material and financial resources have been invested and some achievements have been made. In this paper, retrospective and prospective two methods of Taishan City, Guangdong Province, knot prevention and control stations and Huiyang chronic disease prevention and control of tuberculosis costs were calculated in order to use the results to evaluate the effectiveness of control measures. The average hospital management of tuberculosis prevention and treatment of township hospitals a patient cost of 164.92 yuan. In the four prospective surveys conducted in December 1996 and March, June and September 1997, the cost estimates were as follows: Taishan City’s prevention and treatment of one case of new smear-positive patients was 327.76 yuan, and one case of retreatment was completed Smear positive patients cost 439.73 yuan; and Huiyang City chronic disease station to complete the treatment of a case of new smear positive patients cost 675.27 yuan, the completion of treatment of a case of retreatment smear positive patients cost 787.80 yuan.
