目的 探讨年龄与病程对Ⅱ型糖尿病患者的胰岛素降解速度的影响。方法 对老年Ⅱ型糖尿病患者49 例、非老年Ⅱ型糖尿病患者71 例以及正常对照60 例,检测了红细胞胰岛素酶活性(EIA)、空腹血浆胰岛素(FINS)、空腹血糖(FPG)和糖基化血红蛋白(HbA1c) ,并以FINS和FPG乘积的倒数作为胰岛素敏感性指数(ISI) 。结果 老年及非老年Ⅱ型糖尿病患者的EIA 显著高于正常对照组,其中,老年Ⅱ型糖尿病患者的EIA 和FPG 高于非老年患者,而ISI显著低于非老年患者。相关分析表明,2 组糖尿病患者的EIA 与病程和FINS呈显著负相关。结论 老年Ⅱ型糖尿病患者的红细胞胰岛素降解速度升高,而ISI下降,提示随着年龄增大和病程延长,Ⅱ型糖尿病患者的胰岛素抵抗逐渐加重。
Objective To investigate the effect of age and duration on the rate of insulin degradation in type 2 diabetic patients. Methods Forty-nine elderly patients with type 2 diabetes mellitus, 71 non-elderly patients with type 2 diabetes mellitus and 60 normal controls were enrolled in this study. EIA, fasting plasma insulin (FINS), fasting plasma glucose (FPG) and glycosylation Hemoglobin (HbA1c) and the inverse of the product of FINS and FPG as the insulin sensitivity index (ISI). Results The EIA of elderly and non-elderly patients with type 2 diabetes mellitus was significantly higher than that of the normal controls. EIA and FPG were higher in elderly patients with type 2 diabetes than in non-elderly patients, while ISI was significantly lower than that in non-elderly patients. Correlation analysis showed that two groups of diabetic patients with EIA duration and FINS was significantly negatively correlated. Conclusions The rate of erythrocyte insulin degradation in elderly patients with type 2 diabetes mellitus is increased, while the ISI is decreased. It suggests that with age and duration of disease, insulin resistance in patients with type 2 diabetes mellitus gradually aggravates.
