目的探讨抗反转录病毒治疗(ART)前后,艾滋病病毒1型(HIV-1)感染者外周血中调节B细胞(Breg)和记忆B细胞(Bmem)的平衡,以及白介素-10(IL-10)表达水平与HIV-1感染疾病进展的关系。方法选择30例未治疗的HIV-1感染者,于ART治疗前和治疗12周,分离外周血单个核细胞(PBMC),用流式细胞术(FCM)检测CD19+CD24hi CD38hi B细胞、CD19+CD24hi CD38loB细胞的及CD19+IL-10+B细胞比例的变化;用酶联免疫吸附试验法(ELISA)测定培养上清中IL-10水平,与健康对照组比较。结果相对于健康对照组,感染HIV-1后,CD19+CD24hi CD38hi B细胞的比例(P=0.0001)、Breg/Bmem比值(P<0.0001)及上清中IL-10水平(P=0.0028)均升高,而CD19+CD24hi CD38loB细胞的比例(P=0.0005)显著降低,差异具有统计学意义。ART开展12周后,CD19+CD24hi CD38hi B细胞的比例(P=0.0069)、Breg/Bmem比值(P=0.0012)及上清中IL-10水平(P=0.041)均显著降低,且上清中IL-10水平与Breg(R2=0.1347,P=0.0039)的比例呈正相关性,而与Bmem细胞的比例呈显著负相关(R2=0.1705,P=-0.001)。结论 HIV-1感染改变了CD19+CD24hi CD38hi B细胞的比例、其相关细胞因子IL-10的表达水平,并破坏了Breg/Bmem平衡;经过ART治疗后,CD19+CD24hi CD38hi B细胞的比例与其相关细胞因子IL-10的表达水平及Breg/Bmem平衡可以得到逐渐恢复。
Objective To investigate the balance of regulatory B cells and memory B cells (PBMCs) in peripheral blood of HIV-1 infected patients before and after antiretroviral therapy (ART), and the relationship between interleukin-10 (IL- 10) expression and the progression of HIV-1 infection. Methods Thirty untreated HIV-1 infected individuals were selected. Peripheral blood mononuclear cells (PBMCs) were isolated before and 12 weeks after ART treatment. The expressions of CD19 + CD24hi CD38hi B cells, CD19 + CD24hi CD38loB cells and the ratio of CD19 + IL-10 + B cells. The levels of IL-10 in culture supernatants were determined by enzyme-linked immunosorbent assay (ELISA), compared with the healthy control group. Results Compared with healthy controls, the percentage of CD19 + CD24hi CD38hi B cells (P = 0.0001), Breg / Bmem ratio (P <0.0001) and the level of IL-10 in supernatant (P = 0.0028) , While the proportion of CD19 + CD24hi CD38loB cells (P = 0.0005) was significantly lower, the difference was statistically significant. The proportion of CD19 + CD24hi CD38hi B cells (P = 0.0069), Breg / Bmem ratio (P = 0.0012) and IL-10 level in supernatant (P = 0.041) The level of IL-10 was positively correlated with the proportion of Breg (R2 = 0.1347, P = 0.0039), but negatively correlated with the proportion of Bmem cells (R2 = 0.1705, P = -0.001). Conclusion HIV-1 infection changed the ratio of CD19 + CD24hi CD38hi B cells, the expression level of related cytokines IL-10 and the balance of Breg / Bmem. After ART treatment, the proportion of CD19 + CD24hi CD38hi B cells correlated with Cytokine IL-10 expression levels and Breg / Bmem balance can be gradually restored.
