目的:观察创面和皮缘的角度对愈合的影响。方法:采取手术造模,使皮肤创缘与创面呈90°,称“直切组”。皮肤创缘与创面呈135°,称“斜切组”。分别观察术后3、5、7、14、21、28d病理变化。结果:斜切组,在创伤早期毛细血管数量丰富(P<0.05);纤维母细胞明显丰富(P<0.01);皮缘上皮增生出现早,爬行生长速度快;在愈合全过程中炎性细胞计数显示炎性反应轻;愈合明显快(P<0.01)。结论:创缘与创面角度越大,越利于炎性渗出物的引流,保留较多的基底层细胞对于伤口收缩及肉芽组织中毛细血管、纤维母细胞生长均有促进作用,有利于伤口的愈合,符合李竞提出的“肌(肉芽)平皮(上皮)长、皮长肌平”的皮肤溃疡愈合规律。
Objective: To observe the effect of wound and skin angle on healing. Results: The number of capillaries was significantly increased in the early period of trauma (P <0.05), the number of fibroblasts was significantly increased (P <0.01) Inflammatory cell counts showed mild inflammatory response; healing was significantly faster (P <0.01). CONCLUSION: The larger the wound margin and the wound angle, the more conducive to the drainage of inflammatory exudates. The retention of more basal cells promotes the contraction of the wound and the growth of capillaries and fibroblasts in the granulation tissue, which is good for the wound Healing, in line with Li Jing’s “muscle (granulation) flat skin (epithelium) long, long skin Ping” skin ulcer healing.
