目的观察下咽癌中maspin蛋白的表达,探讨maspin在下咽癌发生发展中的作用及其作为预后指标的意义,并探讨maspin与血管和淋巴管生成的关系。方法应用免疫组化法(SP法)对53例下咽癌组织进行maspin染色,分析maspin表达与下咽癌的组织学类型、临床分期、淋巴结转移和预后等病理特征的关系。分别用CD105和LYVE-1标记新生血管和新生淋巴管,分析maspin表达与微血管密度(MVD)和微淋巴管密度(LVD)的关系。结果 maspin在下咽癌中的阳性率为47.2%(25/53),在不同分化程度和有无淋巴结转移的下咽癌中表达差异有统计学意义(P均<0.05)。maspin表达阳性者中位生存时间(67个月)与表达阴性者(36个月)差异有统计学意义(P<0.05)。在maspin阳性表达的25例下咽癌中,MVD均值为6.56±2.22,LVD均值为10.76±3.37;maspin阴性表达的28例中,MVD均值为8.14±2.46,LVD均值为15.04±4.57。t检验显示,maspin在下咽癌中的表达与MVD和LVD呈负相关,差异有统计学意义(P均<0.05)。结论 maspin表达降低与下咽癌病理分化程度低、淋巴结转移有关。maspin可能对下咽癌中的血管及淋巴管生成起抑制作用。
Objective To observe the expression of maspin protein in hypopharyngeal carcinoma and to explore the role of maspin in the genesis and development of hypopharyngeal carcinoma and its significance as a prognostic marker and to explore the relationship between maspin and angiogenesis and lymphangiogenesis. Methods 53 cases of hypopharyngeal carcinoma were stained with maspin by immunohistochemistry (SP method). The relationship between maspin expression and histopathology, clinical stage, lymph node metastasis and prognosis was analyzed. Neonatal and neonatal lymphatic vessels were labeled with CD105 and LYVE-1, and the relationship between maspin expression and microvessel density (MVD) and lymphatic vessel density (LVD) was analyzed. Results The positive rate of maspin in hypopharyngeal carcinoma was 47.2% (25/53). There was significant difference in the expression of maspin between hypopharyngeal carcinoma with different degree of differentiation and lymph node metastasis (all P <0.05). The median survival time (67 months) of maspin positive expression was significantly different from that of negative expression (36 months) (P <0.05). Among 25 cases of maspin-positive hypopharyngeal carcinoma, the mean MVD was 6.56 ± 2.22 and the mean LVD was 10.76 ± 3.37. Among 28 cases with maspin negative expression, MVD mean was 8.14 ± 2.46 and mean LVD was 15.04 ± 4.57. The t test showed that maspin expression in hypopharyngeal carcinoma was negatively correlated with MVD and LVD, the difference was statistically significant (all P <0.05). Conclusion The decrease of maspin expression is associated with the low degree of pathological differentiation of hypopharyngeal carcinoma and lymph node metastasis. Maspin may inhibit angiogenesis and lymphangiogenesis in hypopharyngeal carcinoma.
