为提高鼻窦真菌病临床诊断和治愈率,对13例鼻窦真菌病了解了生活史和术前进行了鼻窦内镜、CT扫描以及病理、病原菌和变应原检查;对13例均手术治疗,其中6例真菌团做化学成分分析。结果显示,真菌团化学成分主要是锌、铜、铁、镁、钙的磷酸盐。其中以磷酸钙为主,提示CT扫描显示的高密度阴影对该病具有特征性诊断意义。13例随访至今无复发。结论:生活环境、年老体弱、免疫力低下、鼻腔结构异常及鼻息肉等局部因素是鼻窦真菌病发病的主要因素,积极的手术治疗配合抗真菌药是治疗该病的有效措施。手术方法的选择可提高治愈率。
In order to improve the clinical diagnosis and cure rate of sinus fungal diseases, 13 cases of sinus mycosis were acquired, the life history and preoperative sinusography, CT scan and pathological, pathogenic bacteria and allergen examination were performed; 13 cases were treated by surgery, of which 6 cases of fungal group to do chemical composition analysis. The results showed that the chemical composition of fungal groups are mainly zinc, copper, iron, magnesium and calcium phosphate. Of which calcium phosphate-based, suggesting that high-density CT scan shows the disease has a characteristic diagnostic significance of the disease. No recurrence of 13 cases so far. Conclusion: Local factors such as living environment, frail elderly, low immunity, abnormal nasal structure and nasal polyps are the main factors of the pathogenesis of sinusitis. Active surgical treatment with antifungal agents is an effective measure to treat the disease. The choice of surgical method can improve the cure rate.
