[目的]探讨非超踝Taylor空间支架治疗胫骨Pilon骨折的安全性及临床疗效.[方法]回顾性分析2015.2-2017.2手术治疗胫骨Pilon骨折患者共30例,按照治疗方法分为联合固定组与内固定组.联合固定组采用闭合复位Taylor空间支架联合有限内固定进行治疗,内固定组采用传统切开复位内固定治疗.比较两组患者的术中出血量、手术时间、骨折愈合时间和并发症发生率以及骨折愈合拆除外固定支架随访2个月后使用AOFAS踝-后足评分标准(AOFAS ankle hindfoot scale)评估踝关节功能恢复情况.[结果]手术时间、出血量及愈合时间两组差异有统计学意义(P<0.05),联合固定组优于内固定组,并发症发生率内固定组高于联合固定组.全部患者均得到完整随访,骨折愈合拆除外固定支架随访2个月后根据AOFAS踝-后足评分系统(AOFAS ankle hindfoot scale)评估患者躁关节功能恢复情况,其中联合固定组优良率86.67%,内固定组优良率60.00%,差异有统计学意义(P<0.05);[结论]闭合复位非超踝Taylor空间支架联合有限内固定治疗胫骨Pilon骨折可以缩短手术时间,降低并发症发生率,减少骨折愈合时间.是一种易于掌握的、具有较低并发症和临床疗效满意的技术,值得在临床广泛推广.“,”[Objective] To investigate the safety and clinical outcome of non-span ankle Taylor spatial frame for the treatment of tibial Pilon fractures.[Methods] From February 2015 to February 2017,30 patients with tibial Pilon fractures received surgical treatment,including 15 patients with closed reduction,external fixation of non-span ankle Taylor frame combined with limited internal fixation (the combined group),and 15 patients with traditional open reduction internal fixation (the internal fixation group).Operation time,intraoperative blood loss,fracture healing time and complications were recorded and compared between two groups.In addition,AOFAS Ankle Hindfoot Scale was used for assessment of ankle joint recovery.[Results] The combined group was significantly superior to the internal fixation group in clinical results,including less operation time [(60.20±6.81)min versus (112.40±10.08)min,P<0.05],less intraoperative blood loss [(42.73±8.19)ml versus (103.20±9.20)ml,P<0.05],and less fracture healing time [(31.53±3.68)weeks versus (68.40±4.19)weeks,P<0.05],with less complication rate.All patients were followed up regularly till 2 months after frame remove.In term of AOFAS criteria,the patients were graded as excellent and good results in 86.67% of the combined fixation group,whereas 60.00% of the internal fixation group,with a significant difference between them (P<0.05).[Conclusion] For the treatment of tibial Pilon fracture,closed reduction and Taylor fixation combined with limited internal fixation can shorten operation time,reduce complications and accelerate fracture healing.It is an easy technique with low complication and satisfactory clinical outcomes.
