目的采用多普勒超声二尖瓣及肺静脉血流频谱的有关参数,预测慢性心衰患者的肺动脉嵌楔压。方法45名(随机分为实验组35名及检验组10名)不同程度心功不全患者,同时进行右心导管及多普勒超声检查,记录有关参数。然后分别进行简单及多元逐步线性回归分析,将得出的多元线性方程在检验组10名患者中进行检验。结果通过简单的相关及回归分析,二尖瓣血流频谱中的舒张早期前向血流E峰的下降时间DT相关性最好(r=-0.90),其次是E/A(r=0.84)及肺静脉血流中的S/D(r=-0.78)。逐步回归分析得出的多元线性方程包含上述三个变量。在检验组,其估测与实测肺动脉嵌楔压的符合率较好。结论结果表明:对各种心脏病所致的慢性心功能不全,多普勒超声结合二尖瓣及肺静脉血流参数能较可靠地测定肺动脉嵌楔压
Objective To predict the pulmonary artery wedge pressure in patients with chronic heart failure by using Doppler parameters of mitral and pulmonary venous flow spectrum. Methods Forty-five patients (35 randomly assigned to experimental group and 10 in test group) underwent cardiac catheterization and Doppler echocardiography. The related parameters were recorded. Then simple and multiple stepwise linear regression analysis was performed separately, and the multivariate linear equation was tested in 10 patients in the test group. Results By means of simple correlation and regression analysis, the correlation coefficient (DT) of DT in mitral flow was the best (r = -0.90), followed by E / A (r = 0.84) and S / D in pulmonary venous flow (r = -0.78). The multivariate linear equation obtained by stepwise regression analysis includes the above three variables. In the test group, its estimation and measured pulmonary artery wedge pressure in line with the better rate. Conclusion The results show that: a variety of heart diseases caused by chronic heart failure, Doppler ultrasound combined mitral and pulmonary venous flow parameters can be more reliable determination of pulmonary artery wedge pressure
