目的对广州市白云区产科质量相关指标进行分析,为提高产科质量制订相应干预措施提供科学依据。方法回顾性分析广州市白云区2003—2012年共10年间产科质量报表。结果近10年来广州市白云区分娩量逐年递增,增长了184.62%,外地户籍孕产妇占比增幅明显,从2003年的63.03%上升到2012年的84.01%;分娩并发症、新生儿并发症发生率、围生儿死亡率稳步下降,分娩并发症平均发生率从2003年的4.60%逐年下降到2012年的3.30%,新生儿并发症发生率从2003年的2.59%下降到2012年的1.71%,围生儿死亡率从2003年的1.15%下降到2012年的0.45%;产后出血和新生儿窒息分别是产、儿科的首位并发症。结论 10年来广州市白云区通过加强区妇幼保健院在区域产科质量管理中的作用、强化全员培训、采取责任单位问责制等措施,产科质量有所提高。今后需要进一步加强多部门合作、实现信息共享、加强健康教育、提高助产及新生儿窒息复苏等产时适宜技术、加强产后出血团队技能培训等措施切实提高产科质量。
Objective To analyze the relevant indicators of obstetrics quality in Baiyun District of Guangzhou City and provide scientific basis for improving the obstetric quality and formulating corresponding intervention measures. Methods The obstetric quality reports of Baiyun District in Guangzhou from 2003 to 2012 were retrospectively analyzed. Results The delivery volume of Baiyun District in Guangzhou increased by 184.62% year by year in the past 10 years. The proportion of pregnant women with foreign residents registered a significant increase from 63.03% in 2003 to 84.01% in 2012. The complications of delivery and neonatal complications Rate of perinatal mortality decreased steadily, the average incidence of complications of childbirth decreased from 4.60% in 2003 to 3.30% in 2012, neonatal complications from 2.59% in 2003 decreased to 1.71% in 2012, . The perinatal mortality rate dropped from 1.15% in 2003 to 0.45% in 2012. Postpartum hemorrhage and neonatal asphyxia were the first complication of birth and pediatrics respectively. Conclusion The quality of obstetrics has been improved in Baiyun District of Guangzhou in 10 years by strengthening the role of district MCH in the quality management of obstetrics and gynecology, strengthening all-staff training and adopting the accountability system of responsible units. In the future, we need to further strengthen multisectoral cooperation, share information, strengthen health education, improve midwifery techniques such as midwifery and neonatal asphyxia recovery, and strengthen skills training for postpartum hemorrhage teams to effectively improve the quality of obstetrics.
