目的:探讨术前肾血管CT血管成像(CTA)在指导腹腔镜下肾部分切除术中血管处理的作用。方法:15例患者中肾癌9例,肾错构瘤6例。术前均行肾血管CTA。经腹膜后入路施行腹腔镜肾部分切除术,术中根据CTA提示寻找并阻断肾动脉。结果:CTA示肾门前肾动脉分支3例,占总病例数20%,副肾动脉1例,占总病例数6.7%。术中探查均发现CTA所提示的异常血管,探查未发现其他异常动脉。本组15例全部手术成功。结论:CTA能清楚显示血管的解剖及变异,为术中处理肾动脉提供有效的指导。
Objective: To investigate the effect of preoperative renal vascular CT angiography (CTA) on the guidance of vascular treatment during laparoscopic partial nephrectomy. Methods: There were 9 cases of renal cell carcinoma and 6 cases of renal hamartoma in 15 cases. Preoperative renal vascular CTA. Peritoneal approach for laparoscopic partial nephrectomy, intraoperative CTA prompts to find and block the renal artery. Results: CTA showed three branches of renal artery anterior renal artery, accounting for 20% of the total number of cases and 1 case of accessory renal artery, accounting for 6.7% of the total cases. Intraoperative exploration found abnormal blood vessels prompted by CTA, exploration did not find other abnormalities. All 15 patients in this group were successfully operated. Conclusion: CTA can clearly show the anatomy and variation of blood vessels and provide effective guidance for the treatment of renal artery during operation.
