目的:观察在宫颈癌患者同步放化疗后应用拓扑替康联合顺铂与紫杉醇联合顺铂两种化疗方案巩固治疗的疗效及毒副反应。方法:回顾性分析58例根治性放化疗及术后存在淋巴结转移同步放化疗后的宫颈癌患者的临床资料,其中应用拓扑替康联合顺铂巩固化疗18例(A组),应用紫杉醇联合顺铂巩固化疗22例(B组),未进行巩固化疗者18例(C组),治疗后随访3年(36个月),比较3组的疗效及毒副反应。结果:3组复发率差异无统计学意义(P>0.05),3组3年平均疾病无进展生存期(PFS)及总生存期(OS)比较差异无统计学意义(P>0.05)。最常见的毒副反应为中性粒细胞减少症,A组脱发及周围神经系统反应的发生率较B组低(P<0.05)。结论:根治性放化疗及术后存在淋巴结转移的宫颈癌患者同步放化疗后应用拓扑替康联合顺铂及紫杉醇联合顺铂巩固治疗,与放疗后无巩固化疗患者相比,3年PFS及OS无明显差异,拓扑替康联合顺铂化疗副反应轻。
Objective: To observe the curative effect and toxicity of topotecan combined with cisplatin and paclitaxel combined with cisplatin in patients with cervical cancer after concurrent chemoradiotherapy. Methods: A retrospective analysis of 58 cases of radical radiotherapy and chemotherapy and postoperative lymph node metastasis after concurrent radiotherapy and chemotherapy of cervical cancer patients, including the application of topotecan combined with cisplatin consolidation chemotherapy in 18 cases (A group), the application of paclitaxel combined cis Twenty-two patients (group B) with platinum consolidation chemotherapy, 18 patients without consolidation chemotherapy (group C) were followed up for 3 years (36 months) after treatment. The curative effect and toxicity of the three groups were compared. Results: There was no significant difference in the recurrence rate between the three groups (P> 0.05). There was no significant difference in the 3-year average progression-free survival (PFS) and overall survival (OS) between the three groups (P> 0.05). The most common toxicities were neutropenia, and the incidence of alopecia and peripheral nervous system reactions in group A was lower than that in group B (P <0.05). CONCLUSION: Topotecan combined with cisplatin and paclitaxel combined with cisplatin in patients with cervical cancer who underwent radical chemoradiation and postoperative lymph node metastasis are treated with concurrent chemoradiation.
