目的:探讨人尿激肽原酶(HUK)对急性缺血性脑卒中(AIS)患者神经功能及影像、超声学检查结果的影响。方法:选取48例来院诊治,愿意自费使用HUK的AIS患者为观察组,并选择同期不愿自费使用HUK的AIS患者48例为对照组,对照组采取常规治疗,观察组在对照组基础上加用HUK。比较两组治疗前及治疗后7d灌注加权成像(PWI)、磁共振血管成像(MRA)和经颅多普勒超声(TCD)检查结果及NIHSS评分,并于治疗前与治疗后3个月行Barthel指数评定其生活能力及临床疗效。记录治疗后不良反应的发生情况。结果:PWI示病灶侧治疗后两组各项血流参数较治疗前均有所好转,但观察组改善幅度均显著大于对照组;MRA示治疗后观察组有22例存在血管狭窄远少于对照组35例;TCD示观察组治疗后有32例改善,远多于对照组19例;治疗后观察组NIHSS、Barthel及IP体积改善幅度远大于对照组,且治疗有效率也显著高于对照组,而不良反应却与对照组差异不明显。结论:HUK应用于AIS患者能够有效改善神经功能及影像、超声等检查结果,恢复生活活动能力,值得推广应用。
Objective: To investigate the effect of human urokinase (HUK) on neurological function, imaging and ultrasonography in patients with acute ischemic stroke (AIS). Methods: Forty-eight AIS patients with HUK who were hospitalized for treatment at their own expense were enrolled in the study. Forty-eight patients with AIS who chose not to use HUK over the same period were selected as the control group. The control group was given routine treatment. Conclusion: HUK used in patients with AIS can effectively improve the neurological function and imaging, ultrasound and other test results, the ability to restore life activities, it is worth promoting the application.
