目的 探讨白内障超声乳化加人工晶体植入术后排异反应的临床处理方法。方法 对 10例 ( 11只眼 )白内障超声乳化加人工晶体植入术后 ,出现严重排异反应患者予糖皮质激素类药物联合前房清洗术治疗 ,观察临床疗效及并发症。结果 10例 ( 11只眼 )患者排异反应发生后经糖皮质激素类药物 ( 1%百力特眼水 )抗炎 ,抑制免疫反应治疗 1月 ,再联合前房清洗术治疗 ,术后 1d、3d、1周、1月、1年患眼视力≥ 0 3者分别为 2眼( 18 2 % )、8眼 ( 72 7% )、10眼 ( 90 9% )、11眼 ( 10 0 % )、11眼 ( 10 0 % ) ,视力恢复满意 ,且未再发生排异反应。控制排异反应过程中主要并发症有一过性高眼压 ,继发性青光眼 ,角膜内皮水肿。结论 糖皮质激素类药物联合前房清洗术治疗白内障超声乳化加人工晶体植入术后处理排异反应疗效肯定 ,视力恢复迅速 ,并发症少。
Objective To investigate the clinical treatment of rejection after phacoemulsification and intraocular lens implantation. Methods 10 patients (11 eyes) phacoemulsification combined with intraocular lens implantation, severe rejection patients with glucocorticoid drugs combined with anterior chamber cleansing treatment, observation of clinical efficacy and complications. Results After the rejection occurred in 10 patients (11 eyes), the patients were treated with corticosteroids (1% Baitetexieye) and anti-inflammatory for 1 month, then combined with anterior chamber cleansing. After 1d , 3 eyes, 1 week, 1 month and 1 year respectively were 2 eyes (18 2%), 8 eyes (72 7%), 10 eyes (90 9%) and 11 eyes (10 0% ), 11 eyes (100%), satisfactory visual acuity recovery, and no rejection reaction occurred. The main complications in the process of rejection control were transient ocular hypertension, secondary glaucoma and corneal endothelial edema. Conclusion Glucocorticoid combined with anterior chamber cleansing treatment of cataract phacoemulsification and intraocular lens implantation rejection effect of positive treatment, rapid visual acuity recovery, fewer complications.
