目的:观察品质管理圈(QCC)活动对口腔科医务人员手卫生的干预效果。方法:选择口腔科医务人员146例,实施以“提高口腔科医务人员手卫生依从性”为主题的QCC活动,对医务人员手卫生情况进行干预。在医疗护理操作现场观察记录所有入选人员的手卫生状况,对洗手合格及手细菌培养情况进行统计;全体入选人员通过头脑风暴法列举所有可能导致手卫生依从性差的原因,并寻求相应对策,根据其中涉及的所有项目对每例医务人员进行评分。比较干预前后手卫生质量和手卫生依从性。结果:实施QCC活动干预后口腔科医务人员整体洗手合格率72.6%,非常显著高于干预前的39.7%(P<0.01);手细菌培养合格率82.2%,非常显著高于干预前的38.4%(P<0.01)。洗手合格目标达成率117%,手细菌培养合格目标达成率161%。口腔科医务人员整体主动性、责任心、分析问题能力、解决问题能力、协调能力、品质管理手法、自我价值体现和对口腔专业的热爱等8项分值,较干预前均有不同程度提高。结论:QCC活动能够有效提高口腔科医务人员手卫生质量,提升人员自我管理意识与自我管理能力。
OBJECTIVE: To observe the effects of quality management circle (QCC) intervention on hand hygiene of dental staff. Methods: A total of 146 cases of stomatological staff were selected to carry out QCC activities with the theme of “Improving the hand hygiene compliance of dental staff” to intervene in the hand hygiene of medical staff. At the medical nursing operation site, the hand hygiene status of all the selected persons was observed, and statistics on hand washing qualification and hand bacterial culture were collected. All the selected persons brainstormed for the reasons that may cause poor hand hygiene compliance and sought corresponding measures according to Each project involved scored each medical staff member. Hand hygiene quality and hand hygiene compliance were compared before and after intervention. Results: The overall pass rate of hand hygiene was 72.6%, which was significantly higher than that before intervention (39.7%) (P <0.01). The pass rate of hand bacteria culture was 82.2%, which was significantly higher than that of the pre-intervention 38.4% (P <0.01). Hand-washing qualified target reached 117%, hand bacteria training pass rate reached 161%. The scores of eight items including overall initiative, sense of responsibility, ability to analyze problems, problem-solving ability, coordination ability, quality management practices, self-worth and dental profession’s love of stomatology staff were all improved to some extent before intervention. Conclusion: QCC activities can effectively improve the hand hygiene quality of dental health care workers and enhance their self-management awareness and self-management ability.
