Background The envelope mutant of hepatitis B virus blocks the ability of HBV antibodies to recognize single or multiple site mutations in the group “a” epitope of the hepatitis B surface antigen (HBsAg) group. Infected with this HBsAg mutant HBV donor may be missed by the HBsAg screening test and may therefore have an adverse effect on blood safety. Case reports A multiple donor blood donor HBsAg enzyme-positive. HBsAg radioimmunoassay test and four enzyme screening test was negative. In the first enzyme immunoassay, the specificity of the HBsAg response was confirmed by the HBsAg antibody and HBsAg neutralization assay.
