目的 探讨多模态磁共振成像诊断乳腺环形强化病变的效果.方法 回顾性分析2016年8月至2018年2月94例DCE-MRI均表现为环形强化患者的资料,根据病理结果将患者分为良性组和恶性组,比较两组患者的MRI表现、第1分钟强化率、半定量参数早期强化率 (EER) 、Tmax水平及ADC值.建立Logisitic回归模型构建新的联合参数并评价其诊断效能.结果 良、恶性病变组在肿瘤形态、整体边缘、肿瘤直径动态增强时间-信号强度 (TIC) 曲线及DWI扩散受限分布位置方面比较,差异有统计学意义 (P<0.05);良性病变组第一分钟强化率为 (51.67±14.32) %,EER为 (83.96±53.14) %,均低于恶性组,而峰值达峰时间 (Tmax) 为 (130.21±59.44) s,ADC值为 (1.24±0.29) 10-3mm2/s,差异有统计学意义 (P<0.05);强化环形态+DWI扩散受限位置+Tmax+ADC环内ROC曲下面积为0.87[95%CI (0.825~0.916),P<0.05],诊断敏感度和特异度分别为91.47%和88.68%.结论 乳腺MR多模态成像对乳腺环形强化病变有较高的诊断效能.“,”Objective To investigate the diagnostic value of multimodal magnetic resonance imaging on the diagnosis of ring-shaped lesions of the breast. Methods A retrospective analysis of 94 patients with DCE-MRI from August 2016 to February 2018 showed data of patients with ring-enhanced disease .According to the pathological results, the patients were divided into benign group and malignant group. The MRI findings of the two groups were compared. One minute enhancement rate, EER, Tmax level and ADC value. A Logisitic regression model was established to construct new joint parameters and evaluate their diagnostic efficacy. Results In the benign and malignant lesions, the difference in tumor morphology, global margin, tumor diameter TIC curve and DWI diffusion-limited distribution was significant (P<0.05) .The first minute enhancement rate of benign lesion group was (51.67 ± 14.32) %, EER was (83.96 ±53.14) %, both lower than malignant group, and Tmax was (130.21 ± 59.44) s, ADC value was (1.24 ± 0.29) 10-3 mm2/s, the difference was significant (P<0.05); enhanced ring morphology + DWI diffusion restricted position + Tmax+ ADC area under ROC was 0.87 [95% CI (0.825-0.916), P<0.05], the diagnostic sensitivity and specificity were 91.47% and 88.68%.Conclusions Breast MR multimodal imaging has higher diagnostic efficiency for mammary ring-shaped lesions.
