目的初步探讨肥胖与高血压的关系,并测定肥胖型高血压患病者血清脂联素(APN)以及细胞因子TNF-α、IL-6水平。方法选取原发性高血压确诊患者251例为观察组,另选取正常人157例为对照组,对2组临床情况进行调查,测量其体质量、身高及血压,并检测其血中APN、IL-6和TNF-α的浓度。结果在251例高血压患病者中,正常体质量、超重和肥胖者所占比率分别是29.5%、35.2%和35.3%,而在157例健康对照者中,正常体质量、超重和肥胖者所占比率分别是58.6%、29.5%和15.9%。与健康对照相比,高血压患者的IL-6、TNF-α出现明显升高,血清脂联素则呈显著下降(P<0.05)。在102例高血压患者中,超重和肥胖患者与正常体质量的患者相比,其血清中脂联素浓度呈现显著下降(P<0.05),而TNF-α和IL-6无显著性差异(均P>0.5)。结论超重和肥胖与高血压患病有关,此类人群患高血压危险增加。血清脂联素水平降低及细胞因子TNF-α、IL-6的升高与高血压的发病具有一定相关性,具体机制有待进一步观察。
Objective To investigate the relationship between obesity and hypertension and to determine the levels of serum adiponectin (APN) and cytokines TNF-α and IL-6 in obese hypertensive patients. Methods Totally 251 patients with essential hypertension were selected as the observation group and 157 normal subjects as the control group. The clinical condition of the two groups were surveyed, their body weight, height and blood pressure were measured. The levels of APN, IL -6 and TNF-α. Results The prevalence rates of normal body weight, overweight and obesity among the 251 hypertensive patients were 29.5%, 35.2% and 35.3%, respectively. Among the 157 healthy controls, normal body weight, overweight and obesity Accounting for 58.6%, 29.5% and 15.9% respectively. Compared with healthy controls, the levels of IL-6 and TNF-α in patients with hypertension were significantly increased, and the levels of serum adiponectin significantly decreased (P <0.05). In 102 hypertensive patients, serum adiponectin concentrations were significantly decreased in overweight and obese patients compared with those with normal body weight (P <0.05), while TNF-α and IL-6 were not significantly different (P < All P> 0.5). Conclusion Overweight and obesity are associated with the prevalence of hypertension and the risk of hypertension in these people is increased. Serum adiponectin levels and cytokines TNF-α, IL-6 increased with the incidence of hypertension has a certain relevance, the specific mechanism needs further observation.
