目的评价CellDetect 染色技术在子宫颈癌筛查中应用的可行性及价值。方法 CellDetect染色并观察薄层液基涂片172张,与巴氏染色和活检组织病理学结果进行对比,评估CellDetect染色技术应用于宫颈癌筛查的灵敏度和特异度。活检组织染色选自CINⅠ、Ⅱ、Ⅲ级和SCC的病例。结果 CellDetect染色能从颜色上显示出高度病变、低度病变和不典型鳞状上皮细胞,表现为核呈紫红色,而正常的表层细胞表现为细胞质和核为绿色,中间型(副基底细胞和基底细胞)细胞核为绿色或淡粉色;在活检组织中,CellDetect染色于上皮层染色较好,正常组织与病变组织颜色反应分界清楚,对于CIN的分级有很好的帮助。CellDetect染色诊断CIN和SCC的灵敏度、特异度、阳性预测值和阴性预测值分别为85.2%、65.2%、74.2%和78.9%。结论 CellDetect染色对于CINⅡ、Ⅲ和SCC的检出率比较高,与巴氏染色相比,增加了颜色指标,能够区分染色宫颈上皮正常细胞和病变细胞,特别适用于需要临床即时干预的高级别病变的检出,极有可能成为宫颈癌初筛和早期诊断的简单、低价、有效的特殊染色方法。
Objective To evaluate the feasibility and value of CellDetect staining in cervical cancer screening. Methods CellDetect staining and observation of thin liquid-based smear 172, compared with Papanicolaou staining and biopsy histopathological results to assess the sensitivity and specificity of CellDetect staining techniques applied to cervical cancer screening. Biopsies were stained from CIN I, II, III, and SCC. Results CellDetect staining showed a high degree of lesion, low grade and atypical squamous cells in color. The appearance of the nucleus was purple, whereas the normal surface cells showed cytoplasm and nucleus green. The mesothelial cells (parabasal cells And basal cell) nucleus is green or pale pink; in biopsy tissue, CellDetect staining in the epithelial stained better, normal tissue and diseased tissue clear color demarcation, the classification of CIN is very good. The sensitivity, specificity, positive predictive value and negative predictive value of CellDetect staining for CIN and SCC were 85.2%, 65.2%, 74.2% and 78.9%, respectively. Conclusion CellDetect staining has a high detection rate for CINⅡ, Ⅲ and SCC. Compared with Papanicolaou staining, it can increase the color index and differentiate between normal cells and diseased cells of cervical epithelium.
