目的：探索激励式心理干预对初产妇心理、产程、和分娩结局的影响。方法80名初产妇随机均分为两组。对照组接受常规护理；观察组在此基础上又接受探索激励式心理干预，包括心理咨询、资深护士陪伴、Lamaze氏呼吸指导，激励式暗示等。用Hamilton抑郁量表（MADA）、抑郁自评量表（SDS）、Hamilton焦虑量表（HAMA）、焦虑自评量表（SAS）等评价待产期负面情绪。记录产程持续时间、产后出血量、产褥期感染率、新生儿Apgar评分等。结果观察组的HAMD，SDS，HAMA，和SAS记分均显著低于对照组（均P<0.05）。观察组总产程，第一、第二、第三产程持续时间均显著短于对照组（均P<0.05）。观察组产后出血量少于对照组，观察组产褥期感染率低于对照组（均P<0.05）。观察组的新生儿Apgar评分显著高于对照组组（P<0.05）。观察组的产后卧床时间显著短于对照组（P<0.05）。结论激励式心理干预能有效地缓解初产妇的待产期负面情绪，缩短产程，改善分娩的转归。“,”Objective To investigate the effects of incentive psychological intervention on mentality, stages of labor, and birth outcome of primiparae. Methods Eighty primiparae were randomly divided into 2 equal groups: control group receiving conventional care and observation group receiving incentive psychplogical intervention, including psychological counseling, accompaniment by senior nurses, guidance on Lamaze breath, and encouraging suggestions, etc. in addition. Hamilton depression scale (HAMD), Self-rating depression scale (SDS), Hamilton aanxiety scale (HAMA), and Self-rating anxiety Scale (SAS) were used to evaluate the predelivery negative emotions. The duration of labor stage, postpartum hemorrhage volume, puerperal infection rate, and Apgar score of newborn were recorded. Results The scores of HAMD, SDS, HAMA, and SAS of the observaton group were all significantly lower than those of the control group (all P<0.05). The general, first, second, and third labor stages of the observation group all lasted significantly shorter than those of the control group (all P <0.05). The postpartum hemorrhage volume of the observaton group was significantly less than that of the control group (P<0.05). The puerperal infection rate of the observation group was significantly lower than that of the control group (P<0.05).. The Apgar score of the newborn of the observation group was significantly higher than that of the control group (P <0.05). And the postpartum lying-in-bed time of the observation group was significantly shorter than that of the control group (P <0.05). Conclusion Incentive psychplogical intervention effectively relieves the predelivery negative emotions, shortens the length of labor stages, and improves the outcomes of labor of the primiparae.
