目的探讨双吻合器在直肠癌患者全直肠系膜切除(TME)的临床疗效。方法对69例直肠癌患者按照TME原则进行根治性切除的临床资料进行分析,评价双吻合器技术在低位结直肠吻合术中的作用。结果全组均行根治性手术,术后吻合瘘2例(2.9%),吻合口狭窄3例(4.3%),无排尿及性功能障碍。吻合口局部复发5例(7.2%),盆腔软组织复发4例(5.8%),5年生存率为60.9%(42/69)。结论全直肠系膜切除加双吻合器应用是预防直肠癌术后局部复,保持盆腔脏器功能,提高生存率的有效措施,是治疗低位直肠癌的有效方法。
Objective To investigate the clinical effect of double stapler in total mesorectal excision (TME) in patients with rectal cancer. Methods Clinical data of 69 patients with rectal cancer undergoing radical resection according to TME principle were analyzed to evaluate the effect of double stapling technique in low-level colorectal anastomosis. Results All the patients underwent radical surgery. There were 2 cases (2.9%) with anastomotic fistula and 3 cases (4.3%) with anastomotic stenosis without urination and sexual dysfunction. Anastomotic recurrence in 5 cases (7.2%), recurrent pelvic soft tissue in 4 cases (5.8%), 5-year survival rate was 60.9% (42/69). Conclusion The application of total mesorectal excision and double stapling is an effective method to prevent the local complex of rectal cancer and maintain the function of pelvic organs and improve the survival rate. It is an effective method to treat low rectal cancer.
