目的探讨Th17相关炎症细胞因子IL-1b、IL-6、IL-10、IL-12p70、IL-17A、IL-22、IL-23与原发性肝癌的关系,评价其对原发性肝癌诊断的意义。方法收集肝癌组血清标本51例,其中I期肝癌血清标本10例,Ⅱ期肝癌血清标本13例,Ⅲ期肝癌血清标本28例,收集健康对照组血清29例,采用液态悬浮芯片技术进行Th17相关炎症细胞因子的检测。采用独立样本t检验比较各细胞因子肝癌组和健康对照组的差异,应用受试者诊断特征曲线(ROC)得到曲线下面积评估各细胞因子的检测效能。结果分别对IL-1b、IL-6、IL-10、IL-12p70、IL-17A、IL-22、IL-23进行独立样本t检验,7个细胞因子中仅IL-17A肝癌组与正常对照组间差异有统计学意义(P<0.05),其他6个细胞因子肝癌组与正常对照组之间差异无统计学意义(P>0.05);以IL-17A的血清浓度为变量做ROC曲线,其曲线下面积(AUC)为0.882(P<0.05),说明IL-17A作为肝癌血清标志物具有统计学意义,选择IL-17A截断点为11.5μg/ml,约登指数最大,其敏感度为68.6%,特异度为93.1%。结论 Th17相关炎症细胞因子IL-1b、IL-6、IL-10、IL-12p70、IL-17A、IL-22、IL-23中仅IL-17A肝癌组与正常对照组间差异有统计学意义(P<0.05)。ROC曲线分析示IL-17A对肝癌具有诊断价值。
Objective To investigate the relationship between Th17-related inflammatory cytokines IL-1b, IL-6, IL-10, IL-12p70, IL-17A, IL-22 and IL-23 in primary hepatocellular carcinoma Meaning. Methods Totally 51 serum samples were collected from patients with hepatocellular carcinoma, including 10 serum samples of stage I hepatocellular carcinoma, 13 serum samples of stage II hepatocellular carcinoma, 28 serum samples of stage III hepatocellular carcinoma and 29 serum samples from healthy controls. Among the seven cytokines, The difference between the two groups was statistically significant (P <0.05). There was no significant difference between the other six cytokines in the HCC group and the normal control group (P> 0.05). The serum concentration of IL-17A was used as the ROC curve, The area under the curve (AUC) was 0.882 (P <0.05), indicating IL-17A as a marker of liver cancer serum was statistically significant, select IL-17A cut-off point 11.5μg / ml, Youden index maximum, the sensitivity was 68.6% and specificity of 93.1%.
