目的探讨采用人工股骨头置换治疗合并急性脑梗死的高龄股骨颈骨折的临床疗效。方法笔者自2010-01—2014-10采用人工股骨头置换术治疗36例高龄股骨颈骨折合并脑梗死,根据术后Harris评分及X线评估临床疗效。结果所有患者术后随访3~12个月,术后6个月Harris评分:优23例,良9例,可4例,优良率88.9%,术后X线均未出现假体位置不良,随访期间未见假体松动、脱位。结论合并急性脑梗死的高龄股骨颈骨折经过围术期对症处理后,采用人工股骨头置换术安全有效,患者术后可早期下地行走活动,避免长期卧床并发症,提高生活质量。
Objective To investigate the clinical effect of artificial femoral head replacement on femoral neck fractures with acute cerebral infarction. Methods From 2010-01-2014-10, the author used artificial femoral head replacement for the treatment of 36 cases of elderly femoral neck fracture with cerebral infarction. The clinical efficacy was evaluated according to Harris score and X-ray. Results All the patients were followed up for 3 to 12 months. The Harris scores at 6 months after operation were excellent in 23 cases, good in 9 cases and fair in 4 cases, with an excellent and good rate of 88.9%. No postoperative X-ray was found. No prosthesis during the loosening, dislocation. Conclusions Perioperative symptomatic treatment of elderly femoral neck fractures with acute cerebral infarction is safe and effective with artificial femoral head replacement. Patients can walk ambulation early after operation to avoid prolonged bed rest complications and improve quality of life.
