目的通过报道1例经鼻内镜下切除的累及中颅窝的巨大鼻咽纤维血管瘤,回顾并总结该类病变现有的手术方法。方法总结收治的1例有广泛颅底骨质破坏并局限性累及中颅窝的鼻咽纤维血管瘤的临床特点及治疗方法,复习相关文献以了解不同手术方式的特点。结果本患者通过积极的术前准备,在鼻内镜下完整地切除了肿瘤,随访12个月未见肿瘤复发,未出现术中及术后并发症。结论对于累及颅底骨质而颅内侵犯比较局限的鼻咽纤维血管瘤,可以在充分的术前准备的前提下,行鼻内镜下手术切除。
OBJECTIVE: To review and summarize the current surgical procedures for this type of lesion by reporting one case of nasopharyngeal fibrovascular tumor involving the middle cranial fossa removed by endoscopic sinus surgery. Methods One case of nasopharyngeal angiofibroma with extensive skull base bone destruction and limited involvement of the middle cranial fossa was retrospectively reviewed to find out the characteristics of different surgical modalities. Results The patient underwent complete resection of the tumor under endoscopic sinus surgery through positive preoperative preparation. No tumor recurrence was observed at 12 months of follow-up. No intraoperative or postoperative complications occurred. Conclusions Nasopharyngeal angiofibroma, which affects the skull base and the intracranial invasion, may be surgically removed under adequate preoperative preparation.
