乙胺碘呋酮为Ⅲ类抗心律失常药物,主要延长复极及不应期。目前已知,它尚具有其他三类抗心律失常药物的电生理特性:阻滞快钠通道(Ⅰ类),非选择性地阻滞β受体(Ⅱ类);阻滞慢钙通道(Ⅳ类)。此外,它的抗心律失常作用尚由于抑制Na~+-K~+-ATP酶,增加心肌细胞膜通透性及抗甲状腺作用。上述特性不但决定了它的治疗作用,也产生致心律失常作用。
Amiodarone is a class III antiarrhythmic drug that mainly prolongs repolarization and refractory period. It is currently known to have the electrophysiological properties of three other types of antiarrhythmic drugs: blockade of fast sodium channels (class I), blockade of non-selective blockade of β receptors (class II), blockade of slow calcium channels class). In addition, its anti-arrhythmic effect is due to inhibition of Na ~ + -K ~ + -ATP enzyme, increased myocardial cell membrane permeability and anti-thyroid effect. The above characteristics not only determine its therapeutic effect, but also produce arrhythmia.
