目的比较最大强度投影(MIP)和表面遮盖显示(SSD)两种三维螺旋CT门静脉成像(3D-SCTP)重建方法的成像质量,初步评价3D-SCTP对肝硬化门静脉高压患者的临床应用价值。方法对51例患者连续进行了3DSCTP检查。用双盲法评价SSD和MIP两种方法所获3DSCTP图像的成像质量。观察3D-SCTP对门静脉系统的显示情况。结果 MIP在显示门静脉细小分支方面优于SSD(P<0.01);而SSD在显示门静脉与相邻血管间的解剖关系方面好于MIP(P<0.01)。3D-SCTP清楚显示了肝硬化门静脉高压的静脉曲张及门-体分流等表现。结论 3D-SCTP是一种快速、无损伤、有效评价门静脉系统的方法,MIP在显示门静脉细小分支方面优于SSD,SSD在显示三维解剖关系方面优于MIP。
Objective To compare the imaging quality of three-dimensional spiral CT portal vein imaging (3D-SCTP) with maximum intensity projection (MIP) and surface occlusion display (SSD) to evaluate the clinical value of 3D-SCTP in patients with cirrhosis and portal hypertension. Methods 51 consecutive patients underwent 3DSCTP examination. Double-blind method was used to evaluate the imaging quality of 3DSCTP images obtained by both SSD and MIP methods. Observe the 3D-SCTP portal system display. Results MIP was superior to SSD in displaying small branches of portal vein (P <0.01). SSD was better than MIP in displaying anatomic relationship between portal vein and adjacent blood vessels (P <0.01). 3D-SCTP clearly shows varicose veins and portal-to-body shunt in patients with cirrhosis and portal hypertension. Conclusion 3D-SCTP is a rapid, nondestructive and effective method to evaluate portal system. MIP is superior to SSD in displaying small branches of portal vein. SSD is superior to MIP in displaying three-dimensional anatomic relationship.
