8例经手术及病理证实的晚期妊娠伴发子宫内膜异位囊肿的患者,平均年龄29岁,均无不孕史,其中原发痛经7例。孕期中3例合并中央性前置胎盘。剖宫术中见病变主要累及单侧附件,属R-AFS分期Ⅲ-Ⅳ期。1例仅吸出囊液,余行单附件切除或囊肿剥除。术后随访1a以上者6例,0.5a内2例。7例痛经者6例消失;3例再孕;1例原对侧正常附件处有小囊肿在观察中。对妊娠合并卵巢子宫内膜异位囊肿的临床表现、并发症及诊断和处理进行了讨论。
Eight patients with end-stage and endometriotic cysts confirmed by surgery and pathology had an average age of 29 years without any history of infertility, of which 7 were primary dysmenorrhea. 3 cases during pregnancy with central placenta previa. Cesarean section lesions in the main involvement of unilateral attachment, a R-AFS stage Ⅲ-Ⅳ stage. One patient only sucked cyst fluid, the remaining single-attachment resection or cyst stripping. Postoperative follow-up more than 1a in 6 cases, 0.5a in 2 cases. 7 cases of dysmenorrhea in 6 cases disappeared; 3 cases of pregnancy; 1 case of the original contralateral normal attachment at the small cysts in the observation. The clinical manifestations, complications and diagnosis and treatment of pregnancy complicated with ovarian endometriosis were discussed.
