目的 分析血清降钙素原(PCT)和IL-6水平单独及联合检测对急性阑尾炎穿孔的诊断价值.方法 回顾性分析收治的69 例急性阑尾炎患者的临床资料,根据手术后病理报告,分为单纯性阑尾炎组40 例和阑尾炎穿孔组29 例,对两组患者进行血清PCT水平和IL-6水平检测.另选取新入院急性阑尾炎患者26 例为验证组,其中急性阑尾炎穿孔者13 例.对比单纯性阑尾炎组和阑尾炎穿孔组患者血清PCT和IL-6水平;分析急性阑尾炎患者血清PCT和IL-6水平的相关性;对比PCT、IL-6单独检测及联合检测对急性阑尾炎穿孔的诊断价值.结果 阑尾炎穿孔组血清PCT和IL-6水平与单纯性阑尾炎组比较明显较高,差异有统计学意义(P<0.05).急性阑尾炎患者血清PCT和IL-6水平呈正相关,差异有统计学意义(r=0.6124,P<0.05).PCT和IL-6联合检测诊断急性阑尾炎穿孔的灵敏度、真阳性率与PCT和IL-6单独检测比较明显较高(P<0.05).结论 PCT和IL-6对急性阑尾炎具有有效辅助诊断作用,其联合检测的诊断效果明显优于PCT和IL-6单独检测,尤其对阑尾炎是否穿孔具有重要预测价值.“,”Objective To analyze the value of serum procalcitonin (PCT) and interleukin-6 (IL-6) levels in acute appendicitis patients for the diagnosis of acute appendicitis perforation. Methods The clinical data of 69 acute appendicitis patients admitted in our hospital were selected for retrospective analysis, and the patients were divided into the simple appendicitis group (n=40) and the appendicitis perforation group (n=29) according to the postoperative pathological report. The serum PCT and IL-6 levels of patients in two groups were detected. Another 26 acute appendicitis patients newly admitted in our hospital were selected as the verification group, of which 13 cases had acute appendicitis perforation. The serum PCT and IL-6 levels were compared between the simple appendicitis group and the appendicitis perforation group, and the correlation between serum PCT and IL-6 levels in acute appendicitis patients was analyzed. The diagnostic value of PCT and IL-6 alone and the combined detection for acute appendicitis perforation was compared. Results The serum PCT and IL-6 levels in the appendicitis perforation group were significantly higher than those in the simple appendicitis group, and the differences were statistically significant (P<0.05). There was a positive correlation between serum PCT and IL-6 levels in patients with acute appendicitis (r=0.6124, P<0.05). The combined detection of PCT and IL-6 in the diagnosis of acute appendicitis perforation has a significantly higher sensitivity and true positive rate compared with the detection alone (P<0.05). Conclusion PCT and IL-6 can effectively assist in the diagnosis of acute appendicitis. The combined detection of PCT and IL-6 is significantly superior to that of PCT and IL-6 alone, and has an important predictive value for the appendicitis perforation.
