阿狄森氏病(Addison’s disease,通常由自身免疫性肾上腺炎所致)如果诊断和治疗恰当,则预后良好。实际上,只有当发生急性肾上腺危象时,才可能导致死亡。阿狄森氏危象常由病人中途患病引起,与持续呕吐有关,此时阻止了氢化可的松(HC)由胃肠道足够吸收。因此,有人主张肌肉注射HC。如果能够从直肠很好地吸收,则经此途径给予HC可获得满意的自我治疗,而不需反复胃肠外给药。作者对HC栓剂直肠给药的吸收度进行了研究,并比较了直肠给药和常规的肌肉注射给药的HC血浆浓度。
Addison’s disease (usually caused by autoimmune adrenitis) has a good prognosis if diagnosed and treated properly. In fact, death can only occur if an acute adrenal crisis occurs. Addison’s crisis is often caused by a patient’s illness halfway through persistent vomiting, which stops the absorption of hydrocortisone (HC) from the gastrointestinal tract. Therefore, some people advocate the intramuscular injection of HC. Given good absorption from the rectum, HC is given this route with satisfactory self-treatment, without repeated parenteral administration. The authors studied the rectal absorption of suppositories for HC and compared the concentrations of HC plasma for both rectal and conventional intramuscular administrations.
