目的探讨无创正压通气(NPPV)治疗稳定期慢性阻塞性肺疾病(COPD)患者的临床疗效。方法选择急性发作入院治疗好转的COPD患者71例,随机分为无创呼吸机治疗组21例、家庭氧疗组24例、常规治疗组26例,分别治疗1、6个月后观察各组患者血气分析(Pa O2、Pa CO2)、肺通气功能(FEV1%、FEV1/FVC)等指标的变化。结果治疗6个月后,无创呼吸机治疗组Pa O2、Pa CO2、FEV1%明显改善,与治疗前比较差异均有统计学意义(P<0.05)。治疗1、6个月后,家庭氧疗组Pa O2均有明显提高(P<0.05),但PCO2和肺通气功能指标无明显改变;而常规治疗组动脉血气及肺功能治疗前后均无明显变化。结论在常规治疗基础上,无创正压通气可以改善COPD稳定期患者的血气及肺通气指标,其效果优于常规治疗和家庭氧疗组。
Objective To investigate the clinical efficacy of noninvasive positive pressure ventilation (NPPV) in patients with stable chronic obstructive pulmonary disease (COPD). Methods Seventy-one COPD patients who were hospitalized for acute attack were randomly divided into non-invasive ventilator-treated group (21 cases), home oxygen therapy group (24 cases) and conventional treatment group (26 cases). After treatment for 1,6 months, the blood gas Analysis (Pa O2, Pa CO2), pulmonary ventilation function (FEV1%, FEV1 / FVC) and other indicators of change. Results After 6 months of treatment, Pa O2, Pa CO2 and FEV1% in non-invasive ventilator group were significantly improved, with statistical significance (P <0.05). After 1 and 6 months of treatment, there was a significant increase in Pa O2 in the home oxygen therapy group (P <0.05), but there was no significant change in the PCO2 and pulmonary ventilation function indexes. However, no significant changes were found in the arterial blood gas and pulmonary function before and after treatment in the conventional treatment group . Conclusion On the basis of routine treatment, noninvasive positive pressure ventilation can improve blood gas and pulmonary ventilation index in patients with stable COPD, which is superior to conventional treatment and home oxygen therapy.
