目的:探究给予老年2型糖尿病合并高血压患者延续性护理的干预效果.方法:将90例老年2型糖尿病合并高血压患者随机分成对照组与观察组后,对照组实施常规专科护理,观察组实施常规护理+延续性护理,分析二者之间护理效果差异.结果:观察组患者血压、糖化血红蛋白水平均显著低于对照组,P<0.05.观察组ESCA、SF-36评分均显著高于对照组,P<0.05.结论:实施延续性护理可有效改善老年2型糖尿病合并高血压患者疾病情况,同时有助于提升其自护能力与生活质量.
