作者报告1例消炎痛治疗特效的发作性丛集头痛。患者男性,52岁患发作性头痛34年。每于6～20周无症状期后,头痛连续发作3～8周。发作时左眼眶后迅速发生剧烈转动性疼痛、向左颌、齿及颞部放射,伴左眼红、流泪、睑下垂及左鼻孔阻塞感。每次发作持续10～30分钟,最多达20次/日,夜间最严重。除左颈动脉造影发现左前交通动脉有1个小动脉瘤外,其它检查均正常。患者曾用麦角胺、甲基麦角酰胺(methyserg-ide)、氯丙(?)、酰胺眯嗪、可乐宁、苯噻啶、丙咪嗪、皮质类固醇、锂剂及多种镇痛剂,曾拔牙并行左眶下神经离断术及左颈交感神经阻滞术,均无效。
The authors report a case of episodic cluster headache with indomethacin treatment effects. Patient Male, 52 years old Has attacking headache 34 years. Every 6 to 20 weeks after asymptomatic, headache continued to attack for 3 to 8 weeks. The left orbital rapid onset of severe rotational pain, to the left jaw, teeth and temporal radiation, with left eye red, tearing, ptosis and left nostril blocking feeling. Each attack lasted 10 to 30 minutes, up to 20 times / day, the worst night. In addition to the left carotid artery angiography left anterior communicating artery found a small aneurysm, the other tests were normal. Patients have been treated with ergotamine, methyserg-ide, chlorpromazine, amoxicillin, clonidine, phenothiazine, imipramine, corticosteroids, lithium agents and various analgesics. Tooth extraction parallel left inferior infraorbital nerve surgery and left cervical sympathectomy, were ineffective.
