目的 观察右美托咪定对腹腔镜下全子宫切除手术患者CO2气腹时血液动力学以及脑电双频指数(bispectral index,BIS)的影响.方法 60 例ASAⅠ~Ⅱ级、择期静脉全麻下腹腔镜全子宫切除患者,随机双盲分为两组:治疗组(Dex组,n=30)给予负荷剂量1 μg/kg右美托咪定静脉泵注(15 min),后给予维持剂量0.5 μg/kg/h至手术结束;对照组(C组,n=30)同样方式给予等容量生理盐水.记录送入手术室时(T0)、气管插管后1 min(T1)、人工气腹后1 min(T2)及气腹后 30 min(T3)、气腹结束恢复体位后1 min(T4)、拔管后1 min(T5)各时点HR、MAP、BIS值.结果 CO2气腹期间,C组T2~3时HR、MAP、BIS值较T0~1时明显升高(P< 0.05);但Dex组无明显变化(P＞ 0.05).结论 腹腔镜下子宫切除术麻醉中应用右美托咪啶,能产生镇静并降低心率,维持CO2气腹期间血液动力学稳定,同时降低BIS值,减轻CO2气腹应激反应.
