患儿,女,7岁。因皮疹2天发热、腹痛1天入院。患儿麻疹恢复期与水痘病人密切接触,先出现皮肤斑疹,红色,几小时后隆起呈丘疹,深夜形成疱疹,有痒感,疹子从腹部开始向颈、背、头部呈向心性分布。次口高热,持续性脐周隐痛,阵发性加剧。病史中未应用过肾上腺皮质激素及其他免疫抑制剂。 体查:体温39.8℃,脉搏160次,呼吸52次,血压16/12kPa。神清、精神差,躯干、颈、头满布斑、丘疹、疮疹及结痂,眼球结膜、口腔粘膜有丘疹,疮疹直径3～smm无大疱及出血性坏死。双肺呼吸音增粗,叩呈浊音,右肺基部可闻明显湿性罗音,心率160次,律齐,心音低钝,满腹压痛、脐周及右下腹部有反跳痛,肠鸣音活跃,无高调,肝脾未扪及,余检查(一)。
