目的:通过对脑卒中患者工具性日常生活活动能力与其主客观记忆能力之间关系的研究,判别影响IA DL的记忆功能因素,探讨脑卒中患者主客观记忆能力对IADL的预测价值。方法:对52例脑卒中患者用RBMT测试其客观记忆能力;采用视觉模拟评价法评测患者的主观记忆能力;用FAQ评估其IADL能力。然后用Pearson相关分析来观察IADL与主客观记忆能力的关系;再以FAQ为因变量,行多元逐步回归分析和Logistic多元逐步回归分析以探讨脑卒中患者主客观记忆能力对IAD L的预测价值。结果:①患者的主观记忆能力与RBMT呈显著正相关(P<0.05);FAQ与主观及RBMT呈显著负相关(P<0.05,P<0.01)。②多元逐步回归分析显示:RBMT中记被藏物、信封两个分项目以及RBMT总分进入方程(P<0.05),且RBMT总分是IA DL的显著预测因子(R2=0.68,P<0.001),Logistic多元逐步回归分析结果表明,对IADL能否自理的预测,仅有RBMT总分进入模型。结论:脑卒中患者的行为记忆能力对预测其在家庭和社区中的IADL水平有重要意义。
OBJECTIVE: To study the relationship between instrumental activities of daily living in stroke patients and their subjective and objective memory abilities, and to identify the memory function factors that affect IA DL and to explore the predictive value of subjective and objective memory in IADL patients with stroke. Methods: Fifty-two stroke patients were tested for their objective memory using RBMT. Visual analogue assessment was used to evaluate the patient’s subjective memory ability. The FAQ was used to assess the IADL ability. Then, Pearson correlation analysis was used to observe the relationship between IADL and subjective and objective memory ability. And then the multivariate stepwise regression analysis and Logistic multivariate stepwise regression analysis were used to investigate the predictive value of subjective and objective memory in stroke patients. Results: ① There was a significant positive correlation between subjective memory and RBMT (P <0.05). There was a significant negative correlation between FAQ and subjective and RBMT (P <0.05, P <0.01). (2) Multiple stepwise regression analysis showed that RBMT scores of sub-items, envelopes and RBMT scores entered the equation (P <0.05), and RBMT score was a significant predictor of IA DL (R2 = 0.68, P <0.001) ), Logistic multivariate stepwise regression analysis showed that only the total score of RBMT entered the model for predicting whether IADL can take care of itself. CONCLUSIONS: The behavioral memory of stroke patients is important for predicting IADL levels in the home and community.
