近年来为了取得良好的腭咽闭合,在腭裂整复时,常作咽后壁组织瓣转移术。制备咽后瓣时,由于软腭和悬壅垂遮挡,手术野深而狭小,操作困难,常需在悬壅垂尖端穿线作牵引。悬壅垂组织极其娇嫩,术中常因牵引和吸引头的反复损伤,造成水肿或血肿,严重时可整块粘膜剥脱,影响创口愈合,甚至造成该处复裂,穿孔,致使手术失败。最近,我们自制一种悬壅垂及软腭保护器,临床应用效果满意,现简述如下: 一、制作和使用方法: 取1.2mm不锈钢丝,冷弯成支架,上部按腭部生理弧度呈曲线状,下部分两翼,各呈圆弧状,曲面向外,内焊接半圆形白合金片,打磨光滑。(详见图)
