目的了解大连地区HIV-1 CRF 01_AE亚型株的基因特征及其可能传播链。方法对2008—2012年新确认144份全血标本进行血浆分离和HIV RNA提取,巢式PCR扩增Env基因并进行序列测定,并对CRF 01_AE亚型毒株序列进行生物信息学分析。结果得到的HIV感染者有效基因序列中,CRF 01_AE亚型占62.14%(64/103),所有病例均为性接触途径传播。系统进化树分析大连CRF 01_AE存在2个簇,分别以男男同性传播和异性传播毒株为主,其中男男同性性接触人群(MSM)簇组内平均基因离散率小于异性性接触者(Hetero)簇;与其他地区的CRF 01_AE亚型株构建的系统进化树显示2簇内均无其他国家序列,MSM簇与辽宁(除大连地区,下同)、北京和吉林MSM人群的CRF 01_AE亚型株位于同一分支,Hetero簇毒株则与辽宁、北京的MSM流行株及东南沿海地区异性传播CRF 01_AE亚型株遗传距离相近。结论大连市HIV-1 CRF 01_AE株主要通过性接触传播,男男同性传播毒株占主导优势,毒株传入大连时间晚于异性性接触者簇流行株;MSM存在2个以上传播链,与北方部分地区MSM人群中CRF 01_AE亚型株存在多次传播。
Objective To understand the genetic characteristics of HIV-1 CRF 01_AE subtypes and their possible transmission links in Dalian. Methods 144 newly diagnosed whole blood samples from 2008 to 2012 were collected for plasma isolation and HIV RNA extraction. The Env gene was amplified by nested PCR and sequenced. The bioinformatics analysis of CRF 01_AE subtype was performed. Results The effective gene sequences of HIV-infected patients, CRF 01_AE subtype accounted for 62.14% (64/103), all cases were sexually transmitted. Phylogenetic tree analysis showed that there were two clusters in CRF 01_AE in Dalian, which were mainly transmitted by homosexual and heterosexual strains, respectively. The mean gene dispersion rate in MSM cluster was less than that in heterosexual contact (Hetero ) And other regions of the CRF 01_AE subtype strains constructed phylogenetic tree showed no other two sequences within the cluster, MSM cluster and Liaoning (except Dalian, the same below), Beijing and Jilin MSM population CRF 01_AE subtype Strains located on the same branch, Hetero cluster strains and Liaoning MSM epidemic strains in Beijing and southeastern coastal areas heterosexual transmission CRF 01_AE subtype strains close to the genetic distance. CONCLUSIONS: HIV-1 CRF 01_AE strain in Dalian mainly spread through sexual contact, and the male-sexually transmitted strains predominate. The strain spread to Dalian later than heterosexual contact cluster strains. There are more than two transmission chains in MSM, There are multiple transmission of CRF 01_AE subtype in MSM population in some areas in North China.
