目的研究瘦素受体基因rs1137100(Lysl09Arg)和rs1137101(Gln223Arg)多态性位点(SNP)与中国大城市汉族老年人代谢综合征(MS)的相关性。方法根据国际联合暂定声明(JIS)2009年的MS定义,从2002年营养与健康状况调查对象中选取15个省/直辖市18个大城市60岁以上汉族老年人,分为MS组1846人和正常对照组236人,男性701人,女性1381人,共2082人。检测所有研究对象的血浆瘦素和胰岛素,采用Taqman方法检测所有研究对象的瘦素受体基因rs1137100和rs1137101位点的基因型,分析各SNP位点基因型与MS的相关性。结果 MS组BMI、腰围、血糖、血压、甘油三酯(TG)、总胆固醇(TC)、胰岛素、胰岛素抵抗指数(HOMA-IR)、瘦素均显著高于正常对照组,高密度脂蛋白胆固醇(HDL-C)显著低于正常对照组。rs1137100和rs1137101位点GG、AA、GA基因型以及A等位基因频率在两组间分布相似。两个位点AA基因型的SBP和DBP均较高,AA及GA+AA基因型的HDL-c均较低。在rs1137100位点上,与GG型研究对象相比,GA型、AA型研究对象发生MS的风险相似,OR值分别为1.23(95%CI 0.90~1.67)和2.23(95%CI 0.83~6.44);在rs1137101位点上,与GG型研究对象相比,GA型、AA型研究对象发生MetS的风险相似,OR值分别为1.15(95%CI 0.74~1.79)和1.97(95%CI 0.37~10.42)。结论瘦素受体基因rs1137100和rs1137101多态性与中国大城市汉族老年人代谢综合征没有相关性,可能与高血压或血脂异常相关。
Objective To investigate the association between the polymorphisms of rs1137100 (Lysl09Arg) and rs1137101 (Gln223Arg) and the metabolic syndrome (MS) in the elderly in Chinese metropolises in China. Methods According to the definition of MS in 2009 by the International Joint Interim Declaration (JIS), we selected the elderly from the age of 60 in 18 metropolitan areas of 15 provinces / municipalities from the 2002 nutrition and health survey and divided them into 1846 MS group The normal control group of 236 people, 701 males and 1381 females, a total of 2082 people. All subjects were tested for plasma leptin and insulin. The genotypes of rs1137100 and rs1137101 were detected by Taqman method. The genotypes of each SNP loci were analyzed by MS. Results The levels of BMI, waist circumference, blood glucose, blood pressure, triglyceride (TG), total cholesterol (TC), insulin, insulin resistance index (HOMA-IR) and leptin in MS group were significantly higher than those in normal control group and high density lipoprotein cholesterol (HDL-C) was significantly lower than the normal control group. The rs1137100 and rs1137101 loci GG, AA, GA genotypes and A allele frequencies were similar between the two groups. AA genotype at both loci had higher SBP and DBP, while AA and GA + AA genotypes had lower HDL-c. At rs1137100, patients with GA and AA had similar risk of developing MS compared with those with GG, with odds ratios of 1.23 (95% CI 0.90 to 1.67) and 2.23 (95% CI 0.83 to 6.44), respectively ; At rs1137101, the risk of developing MetS in GA and AA subjects was similar to that of GG subjects with ORs of 1.15 (95% CI 0.74 to 1.79) and 1.97 (95% CI 0.37 to 10.42, respectively) ). Conclusion The rs1137100 and rs1137101 polymorphisms of leptin receptor gene are not correlated with the metabolic syndrome in the elderly in Chinese metropolitan cities, which may be related to hypertension or dyslipidemia.
