目的:探讨减少静脉用药调配中心退药的方法,避免药品浪费及成本消耗,保证患者及时用药治疗。方法:对本院静脉用药调配中心2013年8—12月退药单进行统计及分析。结果:2013年8—12月本院静脉用药调配中心共收到输液处方420 000份,其中退药50 358份,占输液处方总量的11.99%。退药原因主要有患者病情变化、经济原因、临时决定出院、转科、死亡等导致更改医嘱或医嘱停用;医生医嘱录入错误或医生查房后未及时调整医嘱等。根据上述情况,重新调整调配中心退药规定,并制定相应措施,如临床药师每月查阅病历审核退药合理性,上报医务科进行考核;医师在退药单上注明原因等措施规范退药。结论:制定静脉用药调配中心管理措施后,有效地减少了退药事件,提高了药物治疗的安全性、有效性、合理性,避免药品浪费及成本消耗。
OBJECTIVE: To explore ways to reduce the withdrawal of drugs from the intravenous drug deployment center, to avoid the waste of medicines and the cost and ensure the timely treatment of patients. Methods: Statistics and analysis were performed on the withdrawal prescription from August to December in 2013 in our hospital's intravenous drug deployment center. Results: From August to December in 2013, a total of 420,000 infusion prescriptions were received, of which 35 358 were withdrawn, accounting for 11.99% of the total infusion prescriptions. The main reasons for the withdrawal of patients with changes in the patient's condition, economic reasons, the temporary decision to discharge, transfer subjects, death, etc. resulting in changes in medical orders or medical orders disabled; doctor's order entry error or doctor did not promptly adjust the doctor's advice and so on. According to the above situation, readjust the regulations of the withdrawal center of the distribution center and make corresponding measures, such as the clinical pharmacists review the rationality of the withdrawal every month and report to the medical department for examination; the physician specifies the reasons on the withdrawal list and other measures to standardize withdrawal . Conclusion: The establishment of intravenous drug deployment center management measures, effectively reducing the withdrawal events, improve the safety of drug treatment, effectiveness and rationality, to avoid drug waste and cost of consumption.
