目的观察牛肺表面活性剂、布地奈德混悬液气管内滴入防治新生儿呼吸窘迫综合征(neonatal respiratory distress syndrome,NRDS)的效果。方法胎龄28~34周,出生2h内早产儿120例随机分为观察组和对照组各60例,对照组气管内滴入牛肺表面活性剂100mg/kg,观察组气管内滴入牛肺表面活性剂100mg/kg+布地奈德混悬液0.25mg/kg混合液,观察治疗3d内2组发生NRDS例数及重复使用牛肺表面活性剂、机械通气情况。结果观察组4例发生NRDS,牛肺表面活性剂重复使用3例,行机械通气治疗2例;对照组12例发生NRDS,牛肺表面活性剂重复使用10例,行机械通气治疗8例,2组比较差异均有统计学意义(P<0.05)。结论气管内早期滴入牛肺表面活性剂+布地奈德混悬液可有效预防NRDS发生,降低牛肺表面活性剂重复应用率、机械通气率。
Objective To observe the effect of intranasal instillation of bovine pulmonary surfactant and budesonide suspension on neonatal respiratory distress syndrome (NRDS). Methods 120 gestational age and gestational age of 28 to 34 weeks were randomly divided into observation group (60 cases) and control group (60 cases). Conclusion The early tracheal instillation of bovine pulmonary surfactant + budesonide suspension can effectively prevent the occurrence of NRDS and reduce the repetitive application rate of bovine pulmonary surfactant and mechanical ventilation rate.
